Does anyone have an itemized list of files and what needs to be in them for a New Entrant Safety Audit?

Driver qualification forms:
1-application for employment (I didn't have this got busted on it)
2-request for check of driving record (DMV record)
3-medical exam report (long form)
4-medical examiners cert
5-record and certificate of road test
6- record of Violations/annual review (he did this before he came)
7- Alcohol & drug test and proof of testing system in place
8-trailer and truck DOT inspection stickers and long forms
9-trucks and trailers maintenance records and your schedule for them
10- log books since you started (he went through 6 months of mine)
11-all insurance forms and who filed them
12-BOC-3 filing form
13-registrations for trucks and trailer
14-Little green book
15-IRPA-IFTA-URC
16-Original DOT and MC filing sheet

Also they can inspect your truck and trailer he didn't bother except to check the GVW stickers on both
